Notes on Loss of Vitreous after Cataract Extraction

چکیده
The subject on which I propose to address you has been long regarded as one in which a satisfactory conclusion one way or other has been considered of the greatest importance in all operations for the removal of cataract. Till recently all the oculists of the United Kingdom and the Continent were unanimous in opinion as to the bad effects of loss of vitreous. This fact is borne out by the records of ophthalmic literature of some ten years ago. Later communications on the subject, however, show the leaning of the profession to a contrary opinion. I am glad to find that the

Prevention of vitreous loss.
Retrobulbar anaesthesia, digital massage, osmotic therapy, and acetazolamide are frequently employed in various combinations to effect ocular hypotony before cataract extraction.* It is believed by many that hypotony prevents vitreous loss during this operation. t Profound akinesia to preclude extraocular muscle contraction and resulting shortening of the scleral shell is a clinically accepted ...
متن کاملAnterior chamber lens implantation after vitreous loss.
Vitreous loss is a serious complication of cataract surgery. Following vitreous loss it is common practice to perform a primary implantation of an anterior chamber lens (AC-IOL). We retrospectively analysed 642 consecutive cases of cataract extraction performed between 1983 and 1986 with special attention to those patients in whom vitreous loss occurred and an AC-IOL was placed. There were 27 s...
متن کاملPearls on anterior vitrectomy for cataract surgeons
With up to 6% of cataract surgeries complicated by vitreous loss,1 anterior vitrectomy is an essential skill of today’s cataract surgeon. Although the incidence of this complication is predictably higher in patients with complex cataracts due to pseudoexfoliation, intraoperative floppy iris syndrome, or posterior polar or dense cataracts, vitreous loss can occur unexpectedly in any case. To dec...
متن کاملCombined vitreous and cataract surgeries in highly hyperopic eye
We report a case of a patient with a highly hyperopic eye who underwent cataract surgery combined with vitreous surgery to create a posterior vitreous detachment (PVD) to prevent choroidal neovascularization (CNV). A 78-year-old man noticed a decrease in his vision due to a cataract in his right eye. The patient had a severe visual loss in his left eye because of a CNV 2 years after a cataract ...

Hyperosmolar solutions and digital massage are frequent adjuncts employed to minimize vitreous loss at cataract extraction (Scott, i962; Virno, Cantore, Bietti, and Bucci, I963; Murray and Tipler, I966; Guyton, I954; Kirsch and Steinman, I955). The former is effective in reducing vitreous mass during the time interval usually necessary to complete a lens extraction, but the latter may not have ...
